Dronabinol im kinderpsychiatrischen Konsildienst der Pädiatrie
In der Pädiatrie ist ein Einsatz von Dronabinol bisher für die Onkologie (Zytostatika-induzierte Übelkeit und Erbrechen und Tumorkachexie), chronische Schmerzen (palliativ und neuropathisch), Spastizität und Restless Legg Syndrom beschrieben. In einer Übersicht des Kinderspitales Datteln, CH wurden pro Jahr ca 4 Patienten behandelt, am häufigsten in der Indikation hypoxisch-ischämische Enzephalopathie und kongenitale Hirnfehlbildung. In Deutschland gibt es eine Publikation aus 2014 zu einer Fallserie, die behandelten Kinder litten unter schwerst- und mehrfach-behinderten Kinder mit Spastik, Schmerzen und Unruhe. 16/17 Kinder erfuhren eine Schmerzreduktion (FLACC-R Scala), 14 eine Reduktion der Spastik (NRS Skala), 12 Kinder eine Verbesserung der Unruhe und nächtlichen Schlafstörung. Opioid Reduktion konnte in 5/9 Fällen reduziert werden, in 2/9 ganz abgesetzt.
Eine retrospektive Analyse von 100 pädiatrischen Patienten des Palliativzentrums des Univ. Klinikums des Saarlandes ergaben bei einer Dosis von 0,1-0,5 mg THC pro kg Körpergewicht zu keine psychotropen Nebenwirkungen und zu keinen klinisch beobachtbaren relevanten Interaktionen bei den Patienten. Dronabinol wird als sicher beschrieben und von den Autoren der Wunsch geäußert, dass dies bei Spastik dem nebenwirkungsreicheren Baclofen als Erstmedikation vorgezogen werden kann. Es fehlen jedoch randomisierte Studien, um den Effekt und die Nebenwirkungen in dieser vulnerablen Population zu quantifizieren.
- Seit der Verschreibungsfähigkeit von Dronabinol seit 1998 ist in Deutschland bisher kein einziger Todesfall durch verordnete Cannabinoide bekannt geworden.
- Wegen der geänderten Cannabis-Gesetzgebung und dem großen Interesse der Patienten wurde dort eine eigene Cannabissprechstunde eingerichtet.
- Seit 2016 werden an den pädiatrischen Stationen der Universitätsklinik für Kinder und Jungendheilkunde Dronabinol in verschiedenen Indikationen verordnet.
- Eine Übersicht dieser Fälle wird den Indikationsbereich, Nebenwirkungen, Wirkung sowie Einstellung der Patienten und ihren Familien und der Behandler darstellen.

Cannabidiol in Pediatric Psychosomatics
Cannabidiol (CBD) is one of the major components of Cannabis, but unlike Tetrahydrocannabinol (THC), it does not have any psychotropic side effects. CBD is a promising substance for the treatment of various neuropsychiatric disorders, e.g. multiple forms of anxiety, trauma, and stress-related disorders or treatment of epilepsy. It reduces anxiety levels in social phobia patients5 and some data even indicates that modulation of the endocannabinoid system may be effective in schizophrenia.
Since CBD has potent anti-inflammatory effects by inhibiting the release of pro-inflammatory mediators (IL-1ß, TNF-α, nitric oxide), it can also be used in the treatment of inflammatory bowel diseases such as Crohn´s Disease or Ulcerative Colitis.
A 14-year old female patient at the psychosomatic unit of Vienna´s General Hospital, had been diagnosed with Crohn’s disease. Additionally, she was suffering from needle phobia, separation anxiety and fear of medical interventions. The patient’s Crohn´s disease was treated with subcutaneous injections of adalimumab, representing a great challenge in the application and resulting in insufficient treatment of Crohn’s disease in this patient.
A first treatment attempt with benzodiazepine remained unrewarding. The patient’s mother declined a therapeutic attempt with SSRI. Therefore, the patient started controlled treatment with cannabidiol in September 2017, after the patient’s mother had already started a self-medication with a cannabinoid substance of unknown dosage and exact composition earlier. E was treated with increasing dose up to a maximum dose of 600mg/d of cannabidiol. After a short-term decrease of dosage during the treatment (between day 127 and 133) to 200mg/d due the holiday season, the patient arrived back at the psychosomatic unit independently to ask for a dose increase because of an insufficient response of anxiety symptoms. The patient reported a significant decrease in anxiety symptoms regarding medical interventions and mood stabilization after being back on 600mg/d of CBD.
Regarding the course of the patient´s Crohn´s disease during the treatment phase, the levels of Fecal Calprotectin (FC), a biomarker that shows the activity of inflammatory bowel diseases11, were analyzed. Because FC levels only showed a marginal decreased under treatment with Adalimumab in combination with cannabidiol, a change of treatment with Ustekinumab instead of Adalimumab was made in January 2018. Consequently, FC Levels decreased from 4741,86 initially to 250,12 in January 2018, indicating a massively reduction of inflammatory activity. After withdrawal of CBD on 23rd March 2018 no particular increase of FC Level was found.
